Personal Release Agreement

For consideration which I acknowlege, I irrevocably grant to Consumed Ministries, Inc. and Consumed Church the right to use my image, name, and/or statement for incorporation into a Consumed product in all forms and media including advertising and related promotion. I have read and understood this agreement.

It's our desire that each KIDsumed, ConsumedNation, and Consumed Connections volunteer understand and agree with the vision and direction of Consumed Church. Therefore, each applicant will be required to meet with the Elder Board to be approved to volunteer in this capacity.
